Eye Rx Database provides an Import Wizard to import your existing patient name and address information into Eye Rx Database.
1. First, you will need to create an MS Excel CSV file containing your patient information. Most other applications allow you to export their data into an MS Excel Spreadsheet or MS Excel CSV format. If you have your patient data in an Excel Spreadsheet, you can resave that Spreadsheet in MS Excel CSV format. In MS Excel, open the Spreadsheet and choose File | Save As menu. Be sure to specify CSV format for 'Save as type'. Note the disk location of your CSV file.
2. Choose File | Import Patients menu and follow the instructions in the Import Wizard
